client eligibility
HRC and TCORE provide services to adults who live in Sacramento County and who meet the county's criteria for having a persistent, mental-health diagnosis as defined in the Mental Health Plan for Sacramento County.
Both agencies are contracted for services through the Sacramento County Mental Health Plan and receive referrals from the Sacramento County Adult ACCESS Team (916)875-1055 or (888)881-4881.

Our Clients must be:
- Adults 18 and over
- Residents of Sacramento County
- Medi-Cal enrollees
- Referred by Sacramento Adult ACCESS Team
- Diagnosed with a persistent, mental-health diagnosis (as defined in Sacramento County's Mental Health Plan)
